The Internal Revenue Service (“IRS”) requires the name, business name, and Federal Employer Identification Number (FEIN) of Innovation Tech Investors, Inc. to be utilized for the completion of the IRS Form W-9 (“W-9”) as it is reported on the Company’s federal income tax return. Interface Systems, L.L.C. is considered a disregarded entity for federal income tax reporting purposes and thus its direct parent entity is reported in Box 1. The direct parent is a C-corporation and an IRS Form 1099 should not be prepared and submitted. The following W-9 forms comply with the IRS instructions:
